Hi, we have a way to do this in place already.

Simply login and click on config>>checkout setup>>options and scroll down to the bottom to

the " how did you hear about us?"

Then Pipe | separate your responses like the example below

Google|Yahoo|Yellow Pages| word of mouth | other

The drop down options appears on the a last page of the checkout.  Change the options to what ever you require.
